League Participation Consent

Prankton Soccer Club participates in youth leagues, festivals, tournaments, and other competitions sanctioned by the U.S. Virgin Islands Soccer Federation (USVISF) and other recognized football organizations. Participation in these competitions is an important part of our player development program and provides valuable opportunities for growth, teamwork, and competitive experience.

Do you give permission for your player to represent Prankton Soccer Club in official league matches, tournaments, and sanctioned competitions throughout the season?

To register your player in the FIFA Connect Registration System and establish eligibility to participate in official competitions sanctioned by the U.S. Virgin Islands Soccer Federation (USVISF), a valid proof of identity is required.

All documents will be handled securely and used solely for player registration, eligibility verification, and compliance with FIFA and USVISF registration requirements.

Media Release & Consent

Throughout the season, Prankton Soccer Club, the U.S. Virgin Islands Soccer Federation (USVISF), tournament organizers, and authorized media partners may capture photographs, videos, and other digital media during training sessions, matches, tournaments, and club events. These images and recordings may be used for promotional, educational, news, and marketing purposes, including but not limited to websites, social media platforms, printed publications, television broadcasts, and other official communication channels.

Do you grant permission for your player to be photographed and/or recorded, and for these images or recordings to be used by Prankton Soccer Club, the U.S. Virgin Islands Soccer Federation, tournament organizers, and authorized media outlets?

Participant Waiver & Release of Liability

Participation in football involves inherent risks, including but not limited to falls, collisions, overexertion, weather-related conditions, and other injuries that may occur during training, matches, tournaments, travel, or club activities.

By registering, I acknowledge that I understand these risks and voluntarily allow the participant to take part in all authorized Prankton Soccer Club activities.

To the fullest extent permitted by law, I release and hold harmless Prankton Soccer Club, its Board of Directors, officers, coaches, volunteers, officials, sponsors, facility owners, and affiliated organizations from any liability for injuries, illnesses, damages, losses, or expenses arising from participation, except where caused by gross negligence or willful misconduct.

I understand that participation is voluntary and that I may withdraw the participant from club activities at any time.

Medical Treatment Authorization

In the event of an accident, injury, illness, or medical emergency during Prankton Soccer Club activities, I authorize Prankton Soccer Club coaches, staff members, volunteers, or designated representatives to obtain emergency medical treatment for my child when I cannot be reached in a timely manner.

I understand that every reasonable effort will be made to contact me or my designated emergency contact before medical treatment is administered whenever circumstances permit.

I agree to assume responsibility for any medical expenses incurred that are not covered by insurance.

I certify that the medical information provided during registration is complete and accurate to the best of my knowledge and agree to notify Prankton Soccer Club of any changes.

I authorize emergency medical treatment for my child if necessary.

Code of Conduct Agreement

Prankton Soccer Club is committed to creating a safe, respectful, inclusive, and positive environment for every participant.

By registering, I agree that the player and family will uphold the values of Prankton Soccer Club by:

Demonstrating respect toward teammates, opponents, coaches, officials, volunteers, and spectators.
Displaying sportsmanship in both victory and defeat.
Following the instructions of coaches and club officials.
Refraining from abusive, discriminatory, threatening, or inappropriate language or behavior.
Protecting club property and facilities.
Representing Prankton Soccer Club with integrity at all times.
Supporting a safe, welcoming, and inclusive environment free from bullying, harassment, or discrimination.

I understand that violations of this Code of Conduct may result in disciplinary action, including suspension or removal from club activities.
